• Nurse Practitioner
• $145/visit
• Travel: Southwest WA Traveler. Covers Clark, Cowlitz, Lewis (Stevenson, Kelso and Chehalis)
• In-home, local travel
• You choose your hours (min. 24/week). Just assessments, no meds, no Rx, no clinic.

• Complete 6–9 in-home health assessments/day
• Review of medical history and risk factors
• Collect vitals, labs, and screening data
• Document findings in EMR
• Educate members on next steps
• Coordinate follow-up if red flags show
• Travel across Southwest Washington (Clark, Cowlitz, and Lewis Counties)

• Must have license/certification: APRN, Certified Nurse Practitioner
• Must be registered with Medicare and/or Medicaid
• Valid driver’s license + reliable car
• Able to work independently in the field

• Choose full-time or part-time hours (min. 24/week)
• Earn $145 per completed visit
• Mileage reimbursed
• Company laptop provided
• Paid training
• No meds, Rx, or call
